Practice Area

Our surgery is located next to Lumbertubs Lower School in Tonmead Road, near to Lumbertubs Way dual carriageway. In accordance with our contractual arrangements with NHS England our practice area currently covers the following list of residential areas and applies only to homes that are situated within the boundary line which follows the route along Great Billing Way, Nene Valley Way, Rushmere Rd, Park Avenue North & South, Kettering Rd, Booth Rise and Talavera Way. 

Existing patients who move home to a new address outside of our Practice boundary are advised to register with another GP Practice that provides General Medical Services and Community Healthcare Services for that area. Please click Find A GP Near My Address

Bus Routes

The no. 16 bus currently provides a frequent public transport service that passes Rillwood Medical Centre along Tonmead with stops situated just a short distance from the surgery premises.

Due to the Coronavirus Pandemic prospective patients wishing you register with our GP surgery are encouraged to do so by completing 1) the `Pre-Registration Form' and 2) `Medical Questionnaire for New Patients' under the `Registering Online' tab.

Applicants will need to provide the following when applying to register:

 1) Proof of a permanent address in the Practice Area and the right to residence in the UK for more than six months (evidence can include a recent history of utility bills or child allowance book, state pension book or original bank statement)

      AS WELL AS

 2) Proof of ID: Passport, Citizen ID Card, Photo driving licence & Counterpart (this necessary in order to verify and ensure that patient medical record is correctly set-up).

New Patient Registration

If you live within our practice area you are welcome to register with us (as a guide to the areas that we cover please click on the above Practice Area tab). Our reception staff will be happy to guide you through the registration procedure.

Eligibility to register with us can be quickly confirmed from your address so please provide proof by way of a recent utility bill, bank statement or state benefit payment book.

In addition we are also required to see proof of your Personal Identity. This is required in the form of photo ID i.e. passport or driving licence photo ID card.

You will need to complete a registration form (GMS1) and also a health questionnaire which will helpfully provide us with background information whilst we wait for your medical records to arrive from your previous doctor.

All new patients aged 40 years and over are offered a health check with a member of the healthcare team to ensure that any routinely required tests are up to date and that we have an accurate note of any repeat medication that you may be taking.

Online access is now available to enable you to view your record, request repeat medications and to book available appointments. Please ask at the practice for more details, and how to register.

Having Trouble Registering?

The NHS Patient Services (Registration) Department for Northamptonshire is now administered from a centre in Leicester and will continue to assist any eligible person who is having difficulty in registering with a local GP medical practice (Tel. 0116 296 7880).

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Please ensure that both the Pre-Registration Form and the Medical Questionnaire are completed before submitting to us.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

If you are visiting from another town and become ill whilst in our area or alternatively if you are not registered with a doctor but need emergency medical attention you will be able to obtain either medical advice or treatment from a local GP practice for up to 14 days. After 14 days you will need to register either as a temporary or a permanent patient in order to continue receiving ongoing medical care.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the practice that is treating you.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment that is clinically assessed as being immediately necessary.

We do not accept temporary registrations from patients who are already registered with another GP Surgery in Northampton.
