Dr Azhar Zafar (Male)

Dr Azhar Zafar is the senior partner for the surgery and is the lead GP for Diabetes, Minor Surgery and Joint injections. Dr Zafar is also the lead GP for research and is actively engaged in academic as well industrial clinical research. He has been working on various research projects on Diabetes as a principal investigator for Northamptonshire.

Dr Zafar is also board member of the Royal College of General Practitioners Thames Valley Faculty.

Dr Kashif Zaman (Male)

Dr Zaman is the practice lead in Quality and Outcomes, Child & Adult safeguarding, Cardiology, Contraceptive services, Child Health and has an interest in minor surgery.

Dr Asim Chishti (Male)

Dr Chishti is the practice lead in elderly medicine, dementia, depression, mental health, palliative care and learning disability and has an interest in minor surgery.

Dr Ayesha Azhar (Female)

Dr Azhar is a GP who also has a special interest and additional qualifications in Gynaecology as well as extensive experience working in hospitals as a Gynaecologist.

As well as General Medical Practice her clinics also include IUD fittings (coil) and contraception implants.

Paramedic
Heena trained as a medical practitioner overseas and in March 2023 joined our medical practice to take up the role of Paramedic. 

The paramedic role involves assessing and triaging patients, including same day triage, and provideing definitive treatment as appropriate including prescribing medications under the supervision of one of our surgery doctors.

Sally is responsible for the strategic management, financial management, staff management, organisation and smooth running of the practice. She has worked for the practice for many years.
 
Sally works with all patient groups and she is also the person to contact for any patient problems or complaints. If you should you have any comments or suggestions on how we can improve our services then please contact her.

Secretaries deal with referral letters, correspondence to patients, medical reports prepared by the doctors and various other administration tasks.

District Nurses (Community)

A team of district nurses provide care services and nursing assessment in the community to housebound patients. Across Northamptonshire since 1 October 2016 a Single Point of Access contact point was set up for all referrals and requests for District Nurse appointments / visits. The telephone number for this service is 0300 777 0002.

With prior arrangement with the District Nurse, wound dressings can be changed at week-ends at the Highfield Clinical Care Centre on Cliftonville Road, Northampton (Weekend Times: 10.30am to 12pm and 1.30pm to 3.30pm).

Health Visitor Community Hub

Health Visitors are qualified nurses with additional training in community health care. They are particularly skilled in seeking out and identifying health needs and work closely with all members of the practice team to promote individual, family and public health.

The Health Visitor works with all age groups; however the majority of clients are families with pre-school children.

Consultations with the health visitor are booked by appointment via the Health Visiting Hub on 0300 1111 022.

Social Prescribing Link Workers

Who is the Service for? Everyone aged 18+ who lives in Northamptonshire.

Who is it not for? Social Prescribing is not for crisis situations. Call 111 if you need urgent assistance or call 0800 917 0464 for mental health emergencies. Strictly speaking Social Prescribing is not a counselling service.
